This infobox template is used for states.
Parameters

Name of the organization
- logo
File name of logo (example: LPNY.png)
- logo-size
Size of the logo (default: 150p)
- type
type of organization

Date of establishment
- phone
- address
- key-people
Name of office holders
- website, facebook, twitter, instagram, meetup
URL of page without the "[" and "]"
Usage
Copy and paste the following to the top of the page you want to insert the template and fill out the information.
